The form must be completed and signed by a health provider (see form below) listing the specific health condition to be managed and the duration of program (not exceeding 12 months).
A claimable Gym Membership is a subscription fee (e.g. per month / per year) that is paid to attend a Gym (ABN required) for the purpose of improving a specific health condition.
A claimable exercise class is an active session to improve the mobility and function of the human body to assist in improving a specific health condition run through a Gym or Personal trainer only (ABN required).
